Method 1:
To add a Visi within the inspection, click the '+' button next to 'Checklist'.
Select 'Defect' and fill out relevant information
Press 'Create and Finish' to publish defects
Method 2:
You could also open SPIF by clicking any child Visi, and hit the '...' button on the top right and hit 'Add Visi'

Press 'Create and Finish' to publish defects
